Eastern Visayas State University (EVSU) acknowledges its obligations under Republic Act 10173, commonly known as the Data Privacy Act of 2012, with regards to the data it collects, records, organizes, updates, utilizes, consolidates, or extracts from students. Your information will be kept in a secure database that is only accessible to EVSU. Rest assured, the information you provide will be treated with the utmost confidentiality.
I have read the University's Data Privacy Statement and hereby express my consent for Eastern Visayas State University (EVSU) to collect, record, organize, update or modify, retrieve, consult, use, consolidate, block, erase, or destruct my personal data as part of my information.
